Mission Statement
The Gallaudet University Athletic Training staff is committed to providing the best health care to the student-athlete physically and mentally while fostering the mission statement and goals set forth by the athletics department. We are determined to help our student-athletes return to participation through the proper utilization of care, prevention, treatment, and rehabilitation.

Sports Physical Forms

     You will need to complete a physical exam and get a physician's clearance for sports if you are planning to join the GU varsity sports team(s). Please download the forms from the link below::

* Sickle Cell Trait (SCT) Test Result
All new student-athletes (including transfers, freshman, students who wish to try-out) MUST present proof of a Sickle Cell Solubility Test in order to participate on any of our intercollegiate athletic teams.
  1.  Submit newborn Screening Test (SCT) Results:
  1. Contact your primary care provider (PCP) or hospital of birth/pediatrician's office and request a copy of your SCT status from birth. Most states required newborn SCT testing beginning in 1990.
  2. Request a copy of your newborn screening information/results from your birth state Dept of Health  cdc.gov/genomics/resources/h.htm
  1. Get tested:
    1. You may use the NCAA sickle cell test site near your home to book a test through quest diagnostics (sicklecelltesting.pwnhealth.com).
    2. You may go to GU Student Health Services (SHS) to get tested if you are already on campus.
 
** Medical Records
Our team physicians and the Athletic Training staff will need to review the medical record you indicated in the history form. (e.g., physician's office notes, surgery notes, cardiology reports, clearance notes, etc.) Please contact your physician's office and obtain those records.
 
     -Returning Student-Athletes
 Please contact:
  • The designated Athletic Training staff to request your required form (medical history form) and set up a concussion baseline test
  • The designated head coach to request online forms
