The Gallaudet University Athletic Training staff is committed to providing the best health care to the student-athlete physically and mentally while fostering the mission statement and goals set forth by the athletics department. We are determined to help our student-athletes return to participation through the proper utilization of care, prevention, treatment, and rehabilitation.

Sports Physical Forms
Student-athletes will need to complete the following checklist in order to participate.
Please contact the designated team's Athletic Trainer to request your forms (see above for contact information).
The checklist:
- Physical Exam and Clearance Adobe Sign form (freshman, transfer, and new only)
- Medical History Adobe Sign form
- ARMS online forms (If you do not have access, please inform your head coach)
- Front and back copies of your health insurance card
- A copy of your COVID-19 vaccination cardÂ
